Patient presents with severe generalised abdominal pain. What's the diagnosis?
Intrahepatic portal venous gas is present. Circumferential wall thickening and oedema of the caecum shows adjacent ileocolic venous gas. The superior mesenteric artery demonstrates a very high grade stenosis at its origin. The findings are concerning for non-occlusive mesenteric ischaemia of the caecum (NOMI).
